I own stock that became worthless last year and need to get that information onto the schedule D and form 8949. I don't have a 1099 How do I do that?

I went to investment income > Add investments > Enter a different way > Stocks, Bonds, Mutual Funds > Continue > Which bank or brokerage is on your 1099‑B? I cannot get past this screen and don't have a 1099-B that reflects this write off.

Enter any name you want, it does not matter and will not be entered on your tax return.
