You do not amend the W-2 from your employer, you prepare a tax return and make payment.
The Ohio Department of Taxation allows you to prepare a taxing school district tax return at this website.
This Ohio Department of Taxation website explains that you should submit an IT-4 to your employer to have school district taxes withheld from your pay.
“Any individual or estate that receives income while a resident of a taxing school district is subject to school district income tax…..To have school district income tax withheld from your pay, submit an IT-4 with the school district name and number to your employer. Please note, your school district withholding is not the same as your city income tax withholding.”
Thank you. I am not amending the W2 but received 3 different versions of my W2. One says "Total City" in box 20 with an added withheld tax in box 19 (total of both city and school district). I received two pages in W2s from one employer. Another version has the city I live in in box 20 with an amount withheld in box 19. The third version has my school district number in box 20 and a different amount in box 19. As mentioned the box 19 amounts in the city version and the school district version add up to box 19 in the Total City version. It, therefore, appears that there was withholding for both the city I live in (and work in) and for the school district. I am not sure which version to use for my federal returns as they all have different info in box 20 and 19. I am assuming I use the Total City version to input but not sure how that then gets broken out for form SD100. Thanks again.
@acicola You're going to use the whole thing for federal so that it will carry over to your state and local returns. When you enter your W-2 it will give you an option for 'state' and even allow you to enter extra states if your employer with held taxes for you in multiple states. And then it has a set of boxes for 'Local Taxes'. The first set of these boxes is your city then click the 'Add another locality' button and enter the info for your school district. The total of these should add up to the 'Your Total City' amount on the one copy you have and everything should be good.
