I live in MA but my company who I have health insurance through is based in NY. Do I still need to submit 1099-HC if I haven't received one?

Please contact your health insurer and ask them if they plan to send you a 1099-HC.

But if they won't, then your employer is responsible to send you the 1099-HC.

This sounds like a good question for your HR department.
