I have e-filed and completed an installment agreement request online. What forms do I need to mail to the IRS?

If you are filing electronically, you do not need to mail the Agreement. And you need to mail a check along with 1040-V as an initial payment if you opted for e-file. First payment will not be deducted from your bank. After your payment plan has been accepted by the IRS, since you gave them your bank information, they will debit the remaining payments on the scheduled dates. If your plan is not accepted you will have to send the remaining payments as checks.

If you file by mail you are required to send the Agreement, 1040-V and check.

No, you absolutely should NOT file an amended return to remove Form 9465.  It is not an actionable form that impacts the actual Form 1040, and once you have set up an installment agreement online, that will override the Form 9565, SHOULD the IRS even deign to attempt to process a Form 9465 submitted with a return (DESPITE instructions to include it, they rarely process them, especially when submitted with the tax return, electronically!)
