I have an incomplete 1099-B from my investment manager who does not have the missing information. No Date Acquired, no quantity sold, no basis. How to fix this?

The amount is small, less than $100, and addition info says Cash In Lieu.

Ok ... the purchase date is the same as the sale date and the cost basis is zero since this was a sell of a fractional share due to a stock split.
