Its an 8962, not 8965. Did you have health insurance through the Marketplace (common referred to as Obamacare)? or did you sign up at some point but didn't keep it? If you had marketplace health insurance, even if for just one month, you would have been issued a Form 1095A. You enter this 1095A into the program and it completes a Form 8962.
