I filed my 2018 taxes in 2022 and received a refund in 2022. How do I enter this on the 2022 form in turbotax?

I've tried entering it in the State and Local Refund section but it doesn't allow me to label it as a 2018 refund.

A 2018 state refund you received in 2022?  I don't think you need to enter the year at all.  Doesn't matter what year it's for.  But you might not need to enter it at all.

A State Tax Refund is taxable if you itemized deductions on that prior year's federal return and took a deduction for state income taxes instead of the sale tax.  You got a deduction benefit for it so now you have to include it as income. If you took the standard deduction it is not taxable and you don't need to report it.
