If you haven't filed yet, step through the File section of TurboTax until you reach the screen How do you want to pay your federal taxes? Select the installment payment plan option, click Continue, and follow the onscreen instructions. (Please see this in the instructions below)

Well - I selected to set up a payment/installment plan.  And it shows 4 vouchers on my paperwork (which is my total amount due divided by 4) BUT does not show my Form 9465. Now I used a different Tax Preparation Service last year and they DID include my Form 9465.  I am very discouraged because I carefully prepared my taxes going through each section and kept looking for the section to take me to fill out the form.  UGH UGH UGH

@stwentier Are you talking about four vouchers that say 1040ES?   Those are not vouchers to pay your current 2021 tax due.  Those are vouchers to pay estimated quarterly payments for 2022 tax ahead of time to avoid an underpayment penalty at tax time next year on your 2022 return.   Unless you want to pay the optional estimated 2022 payments do not use those vouchers.
