I did not receive any G form from anyone.

Do I have to claim my work/Comp disability that is tax free?

Worker's Compensation benefits aren't taxable and aren't entered on your tax return.

 

Depending on the type of disability payments you received determine if you need to report the income on your tax return.  If you received a tax form, then you need to report it on your return under the Wages and Income Section.
