I am filling out the 1099-misc but my form that I received does not have a state listed in box 17. What should I do?

If there's no dollar amounts in Box 16 then you need no state name in Box 17. 

Are there any dollars in Box 16?  If there are then ask the issuer what state got the withheld taxes.
