How to report form 5498 SA / 8889 HSA ?

Does Box 2 include both employer and employee contributions?

If so, how do I report it on Form 8889 HSA?   HELP! 😟

Are you preparing your return by hand?  Download the instructions for the form.  Line 2 is only your own direct out of pocket contributions.  Employer contributions (including your own contributions made by voluntary payroll deduction) go on line 9.
