How to deal with multiple 1099-r's

Hi everyone, in 2022, I got three 1099-r's for the following:

  • IRA backdoor (traditional IRA to Roth IRA)
  • 401k mega backdoor (after tax 401k to Roth 401k)
  • And also rolled over a previous IRA of consolidated 401ks into my current company's 401k
I think I entered all the info correctly in TurboTax, but noticed the following on my Federal and state forms:
  • Line 4a IRA distributions: Backdoor + rollover into current company's 401k with a label that says "rollover"
  • Line 5a pensions and annuities: Mega backdoor 401k
Did I miss something or does that look right? I'm also assuming that form 8606 is for backdoor only.

Thanks everyone!

That looks right.  The only issue is that TurboTax fails to include the word ROLLOVER on line 5b when the taxable amount of the rollover from the 401(k) to the Roth 401(k) is zero, a known problem that has persisted for several years and nobody seems to care about.
