How should I file if I'm separated but not legally separated? We live in two different states and were separated for all of 2018.

If you are still legally married as of December 31, 2018, your only options to file are either as married filing separate or married joint.

If you have a child, step-child or foster child as a dependent that lives with you, then you might also qualify for Head of Household filing status.
