How do I record that I had Medicaid but not a 1095-A?

It is not required. If you had only Medicaid, there is nothing to report for  your healthcare on your tax return.

You do not indicate on your tax return the type of health care insurance.  Since you have Medicaid, when asked if you have a Form 1095-A you answer is NO.
