How do I claim a Dividend check I receive from a UK company? How do I claim this and what type of dividend is it Ordinary or Qualified?

I received no 1099-DIV form this broker.

You can fill out a blank 1099-DIV using USD as if it came from a domestic broker. UK dividends are qualified.
