You will have to contact the previous employer to receive a copy of the W-2 which you did not receive.
You may also use the IRS Get Transcript tool to request your wage and income transcript. It shows the information reported to the IRS on forms W-2. However, the wage and income transcript does not report state and local information. See this IRS website.
