Hi, I have received two W2s , one is from my employer , the second one is from an Agent of my employer. In it is only Box 1,13 (3.p.s.p.),15, 16. Do I add this W2?

Hi, I have received two W2s , one is from my employer , the second is from a company I have never heard of but it says on the w2 that is a Agent of my employer. I have been on a Short-Term Disability Leave because of pregnancy, I am assuming it is the money I got from this program. This W2 is very empty only i Box 1, 15,16. and on Box 13 it is marked "3rd party sick pay",. My question is Do I add this W2 in my 1040 same as I add for my employer?

Yes, this is a separate W-2 (to report sick pay only) - you need to report it on your tax return.

 

Your sick pay is subject to income tax (why box 1 has a value), but it is not subject to Social Security and Medicare tax (this is why boxes 3-6 are blank).
