The employer should be able to provide you a new copy of the W-2. Call or email them.

If you have not yet filed, look in your documents for Form W-2 Wage and Tax Statement. You may have to pay for the software first before you can view your documents. 

If you have already filed, then look at your saved documents for the statement.
