Help entering a Canadian NR4 on my US tax return

Hi,

 

Last year I had to take some fund out of a canadian RRSP.  I now need to enter a NR4.  I have  16 (Gross Income) and boxes 17 (Non-resident tax withheld).  I am not exactly sure where to enter the amounts.  I read I should enter SSA-1099, but when I go to enter the box says (Net income)?  Not sure if I am using the right box.  Can someone provide me with some up-to-date, step by step instructions.
