Hello. I received an employee referral bonus from a company I do not work for. They did not send me a 1099-MISC or any other form. How do I report this?

I tried to choose 'other income not already reported on a W-2 or 1099', but the bonus was larger than $2200 and the company was not my 'employer'.

Actually I think I can go ahead and report this under 'other reportable income'. Is this correct?
