From Jan 2017, I was on PPO policy, in October 2017 I have changed my Job and also health insurance to HDHP plan. How to mention this situation while filing my 2017 Tax?

I am on PPO until October 2017 later I changed my Job and health insurance to HDHP policy, For the last 3 months I have few HSA contributions also, 

I got 2 different 1095C forms from my employers. 

How to handle this situation while filing my 2017 taxes, Please advise.

If there were no gaps in your health insurance coverage but just a change from one plan to another, select I had health insurance coverage all year during the Health Insurance Interview. You don't need to report the information from the 1095-C forms on your tax return but be sure to store them with your records.

If all of your HSA contributions were made with payroll deductions, they'll be reported on your W-2 in box 12 with code W. Only enter this information in the W-2 section of TurboTax. (If you made an HSA contribution that wasn't reported on your W-2, see the first FAQ below.)

If you took money out of your HSA account in 2017, you should have received a Form 1099-SA to report the distribution. You'll need to enter this information in TurboTax (see the second FAQ below).
