Form OMB No. 1545-0008, where do I entered the amount on the return?

That should be a W2.  Doesn't it say W2 in the upper right?

To Add a W2 enter it under

Federal Taxes on the left side

Wages & Income  

Then Wages and Salaries- Click the Start or Update button

Oh it says W2 at the bottom left
