Form 8995 Included, but NOT Needed

Form 8995 is being included in my tax forms, and I cannot figure out why.  I don't have any 1099-DIVs.  It is showing a pretty significant negative value in Lines 3 and 16.  My taxable income is appearing in Lines 11 and 13.  And 20% of my taxable income is showing in Line 14.  Why?  I do have a 1099-G this year, which is new, a 1099-R and 1099-INT; but I don't think these should cause this form to be initiated.  What am I missing?

Lines 3 and 16 relate to carryforward. You have the form due to the carryforward.

 

Your income on the other lines is just the form calculating. See About Form 8995, Qualified Business Income Deduction Simplified Computation.
