You do not need to use the Form 4852, which is used for U.S. income from a U.S. employer who failed to give a W-2.

In Turbo Tax, do not attempt to enter your foreign income in the W-2 section, as it will steer you in the direction of the Form 4852.  Instead, look for the Foreign Income section. It will give you several options of how your income was reported to you, and select "A statement from my foreign employer (could be cash)."
