For box 12 code there is a D 22 and D code. Do I combine the two of them, leave out the D 22 or try to submit as two D codes?

You should enter your W2 as is.  Those are 2 different codes for 2 different items.

 

Code D – Elective deferral under a Section 401(k) cash or arrangement plan. This includes a SIMPLE 401(k) arrangement. You may be able to claim the Saver’s Credit, Form 1040 Schedule 3, line 4. See Form 1040 Instructions for details.

 

Cod DD - Cost of employer-sponsored health coverage. Use this code to report the cost of employer-sponsored health coverage. The amount reported with code DD is not taxable.

I can only put in code D (no D 22).  Keeps flagging as duplicate entry.  Do I just keep over riding it and use D code twice?

Box 12 code D is for Contributions to your 401(k) plan (including a simple 401(k) plan).

Code D 22 is for year 2022. 

You can combine them together or ignore D22.

If combing the D amounts or even leaving off the D 22 would that trigger something on the IRS from what is reported versus what they receive from employer?

Ah, thanks for the added explanation.  By including the amounts in Box 12 code D, You may be able to claim the Saver’s Credit, Form 1040 Schedule 3, line 4. See Form 1040 Instructions for details.  Thus lowering your tax liability.

If 12 a and b are on W2 do I still have to enter on return as deductions

You have to enter the the codes and the dollar amounts in box 12 on the W-2. They may or may not be deductions - just enter them as is.
