Double taxed- Had sale of stocks from non-qualified ESPP plan. Employer didn't put box V on W2 but added $ to box 1. How do I figure $ to adjust cost basis on form 8949?

I would ask the employer for the basis it you didn't get or don't have any paperwork with this info on it.
