How to replace a license or permit
If you use the online option it does not ask for your license number. (Note that the online replacement will be unavailable for scheduled maintenance tomorrow, Sunday, July 26, 2020 from 7:00 AM to 8:30 AM.) If you are filling out the MV-44 application to apply by mail, the instructions under "By mail" at the link above say to enter your name and date of birth if you do not know your ID number.
