DOES A BONUS FOR OPENING A DEPOSIT ACCOUNT AND ITS REPORTED ON A 1099-MISC. DOES IT REQUIRE AN INTENT TO EARN INCOME AND BE REPORTED ON A SCHEDULE C

No, it is "other income".  It should be in box 3, and you would answer the intent questions as "no."
