Do qualified dividends get taxed on both Schedule B (as part of Ordinary Dividends) and also on the Qualified Dividends and Capital Gains Tax Worksheet?

Qualified dividends are not taxed on a Schedule B.  

The dividends are included as part of your taxable income.  The taxable income is the starting point for the taxes being calculated on the Qualified Dividends and Capital Gains worksheet.
