Do I need to write my info (SS, Name, etc) in my supporting statements?

Since I live in a foreign country and I am Self-Employed, I am required to include a photocopy of my foreign Social Security certificate of coverage at the end of my return as a supporting statement in order to avoid double taxation. This Certificate has two pages, first includes all my info (Foreign SS number, US SS number, name, address, etc.) however, the second page only contains the period of coverage and the date, stamp and signature of the Foreign Social Security authority who issued the certificate. 

 

Here my question: Do I need to type my information anywhere in the statements? ( I have 2 separate statements since I am filling married jointly with my husband who is in the same position), additionally I am not sure if I should staple together the 2 pages of each statements.

You should write, or type, all of the pertinent information on any supporting documents in the event the statements become separated.
