Do I have to enter my spouse's W2 if she made less than $5000?

Yes, all income is reportable. Income reported on a government document, like a W-2, is especially reportable.

Although there is a filing threshold, based on total income ($20,600 for a married couple); there is no filing threshold for individual pieces of income. For example, if you had $1 of interest, it too would have to be reported.
