Copy of Consent to Disclosure Form

Is it possible to get a copy of the consent to disclosure form you sign after you progressed passed the screen? I was unable to print the form and was forced to progress to the next step. Was hoping to get access to a copy for my records.

You can download a copy of the consent form along with the completed tax return by going to the print center on the left-side menu under Tools:

  1. Select Print Center, then choose
  2. Print return and all worksheets forms
