If you can enter both then do so. However if entering the one with zero does not affect the outcome only enter the one with values if it clearly describes your insurance situation for 2021. Be care to consider whether the full year is reflected on the form that you enter.

Purpose. Form FTB 3895, California Health Insurance Marketplace Statement, is used to report certain information to the Franchise Tax Board (FTB) about individuals who enroll in a qualified health plan through the California Health Insurance Marketplace (Marketplace).
I am having the same issue with this and did not see a resolution from TurboTax. Please advise.

If your 3895 has zeroes on it do not enter it into TurboTax. It does not effect your taxes.
Using desktop premiere. Under "Monthly Advance Payment of Premium Assistance Subsidy," entering 0 produces an error. Entering 0.00 produces an error. Entering nothing at all (blank) produces an error. In other words, if a user received 0 subsidy each month, Turbotax crashes and burns, and I can't file my taxes. WTH??
