Can I add a signature to the disclosure content form or do I have to?

I believe you are referring to Form 8275 Tax Disclosure. There is no place to enter a signature on that form. Can you clarify better what you are trying to do? It gets filed with your original or amended tax return, so there is no need to sign it separately. Form 8275 Tax Disclosure statement
