Can an 8822-B simply be submitted with my tax return when I e-file, or does it need to be printed and mailed separately?

Form 8822-B lists the address to mail the form to at the bottom of the page.  The addresses listed are different than where the tax return is sent.  It seems that the form should be sent separately from the return itself.
