Can a personal Social Security Number be used for the Employer ID (EIN) to complete the form 9089(Labor Certificate)

No, with employees you will need an EIN.

Thanks! we are applying for the Labor Certificate for child care worker, my boss is a physician, he doesn't have business. I don't know if he can apply for the EIN number.

Employer identification number (EIN).   You must include your EIN on the forms you file for your household employee. An EIN is a nine-digit number issued by the IRS. It isn't the same as a social security number (SSN).  This is under "What Forms must I file" at IRS link below:

I got it. I tried to do it by myself but it works from Monday to Friday. Is it "sole proprietor right? I read the options to apply for the EIN number, then I need his social security numbers.

Ok I have the EIN number now I need to send something of these items:
Proof of business entity:
 
Certificate of good standing
Articles of Incorporation
Certificate of Existence
Business License
State Registration
Official and/ or government documents

Like I said before,My boss doesn't have a business, he is a physician. Can he send his license as professional? Thanks!

You do not send any documentation to the Department of Labor (DOL) unless specially asked for it.  If they need it they will ask you about it.  You only complete the Form 9089 fully.

OK, then send them exactly what they ask for.  Tell them this is for a household employee, not a business.  His professional license should not be needed.  Do not send originals, only photocopies of documents.

Yes, you are right. I already read this note:  HOUSEHOLDS need only submit documents from the "Proof of FEIN", "Proof of Physical Location", and “Proof of Employer Contact” categories.  Please use documents issued within the last 12 months. So I have EIN number ( proof of FEIN); Tax records (PROOF OF PHYSICAL LOCATION). I just need proof of employer contact
