Your employer put the CASDI in the wrong place. That belongs in box 14. Box 15 is for the state ID and the employers state tax ID number only.
Just enter that into box 14 and not box 15 - it will make no difference and does not require a corrected W-2 from your employer.

so i can disregard the CS in box 15, and the 2nd amt in box 16 and 17?, just put the amt from the 2nd line in box 17 in the 2nd line in box 14, right? thanks!!
Yes. Just leave that line (15-17) all blank and enter the CASDI and the amount in box 14 - CASDI only matters if you itemize and deduct state tax. If you do not itemize then it does not matter if you enter it of not.
You can talk to you employer about putting the California State Disability Insurance (CASDI) in the wrong box. The W-2 rules say to enter it in box 14 (at one time employers put it in box 19 and some still do, but it should be in box 14).
