Box 1, 3, and 5 are all the same amount. I had pre tax ded for health ins and 401k. Shouldnt box 1 be less due to those pretax items. Ive never had this happen my wages are always less than ss and medicare wages

Yes, you are correct.

Box 1 should reflect that those deductions were taken out pre-tax. Speak with your employer (if you have not already) and point out the error.
