AZ rejected my form 321 because it says I have a wrong code, but I do Not. How do I fix this?

There are Codes for Arizona Form 321 that apply to donations made by 12/13/18 and different codes that apply to donations made between 1/1/19 and 4/17/19.

Here's a link to look up the correct Code for your donation:

When I put the code in the software says invalid. I have verified the code is correct. Is there something up with the codes in your system?

There's no indication of a software issue.

Please see the Arizona Department of Revenue's Contributions to QCOs and QFCOs page for the latest information.

Yes. The code I have matches the list, and I have verified on the charity page as well. I have tried other codes. It doesn't seem to work with any code/charity.

A common reason for the error message is that name doesn't match exactly with the charity list.  Be sure to check the name and number match and that you are using the list for your contribution time.  For example, if you donated between Jan and Dec 2019 there is a list versus if your donated in 2020, another list.
