A former employer failed to withhold tax form my final pay. Are there any possible negative effects of this other than hosing my AGI? Such as SS wages not being repoted?

Please clarify. Is your final paycheck for the total amount of your pay and no federal, state, Social Security and Medicare taxes weren't taken out? On what form did your final pay appear on at tax time, a W-2 or a 1099-Misc form?
