1099-R

I withdrew early from my retirement account and received a 1099-R, do I list this as a military pension since it was from military earnings? And for state pension type should I also list it as military?

Under present law, a taxpayer who receives a distribution from a qualified retirement plan prior to 59½, death, or disability generally is subject to a 10% early withdrawal tax on the amount includible in income, unless an exception to the tax applies.    There is an exception to the 10% penalty for active-duty members.
