1099-R issued from Florida, no state payer's state number in Box 15, because no withholding; Utah requires a number. What do I do?

No withholding on my Florida pension; no state ID number in Box 15 on 1099-R; cannot file electronically without this number. What is my next step?

try leaving boxes 14-16 blank on the 1099-R
