1099-R Did not receive this form from my financiall institution

Did anyone NOT receive their 1099-R form from their financial institution I did not and was penalized with interest charges by the IRS. The forms were submitted to the IRS but not to me. I had to pay the penalties and interest plus interest on the penalties to the IRS. Did anyone else have this problem? Curious if this is a systemic problem especially concerning an annuity. I would appreciate any response.

You need to take that up with the company sending out the 1099R.   Better double check your address they have for you.  Maybe they expect you to log in to your account and download the 1099R pdf.

contact the FI to get an explanation. The reason doesn't matter to the IRS. From its point of view, you must have known you received this money so while you did not receive the form it was your responsibility to follow up. it could be it was issued but got lost in the mail.

@billtepper

OR, do they provide them to you within the provider's access website, and you may have been mistaken to think they were coming by mail.

Two of my 1099-R forms no longer come to me in the mail, since I'm set up to get them from my 1099-R provider's website electronically, and it's up to me to log in to download them instead.  Perhaps you are set up to get them that way also.

That's what they try to tell me. We did not receive any electronic or digital 1099 - are forms on their website or access to any of our accounts. Looking for other people that had this problem

Call the provider and ask them about it. There should be a contact telephone number in your account website.

They may be able to tell you how to access that form online if you don't see how...or they can send you a new one in the mail...or perhaps explain what happened.   Like...maybe you moved to a new address, and they sent it to an old address...or just plain lost in the mail. That happens too occasionally.

Sure, any number of people miss getting their 1099-R forms for any number of reasons...then they have to take active steps on their own to get those 1099-R forms before filing their taxes.   If you received $$ from a retirement/pension account during 2022, you should have been looking for that 1099-R to put into your tax return. And when you don't get one by (say) the first or second week of February, you should be pestering the 1099-R provider as to where it is long before you file your tax return.
