Why the end of year HSA account value(balance) appeared as taxable distributions at the HSA summary page?

You should go through all your HSA entries again to be sure they are correct.   It could be another entry in this section producing this outcome.

Go through the entire section to be sure data is flowing properly to the HSA Summary. 

When trying to re-create your issue, I used $1000 contribution and a $500 distribution for medical expenses.

The HSA Summary reports correctly the $500 distribution and the $1000 contribution.  

It does not show the $500 account balance as a Taxable Distribution (screenshot).

Looks like I got a confused by wording in the Q - "Did (name) inherit this HSA?". I should answer NO.
