Why is my unspent HSA added as additional income

My unspent HSA contribution is $7500 and it was noted as additional income and increased my taxable income?  This has never happened in the past.

In order to correct this, after you enter your 1099-SA, you'll be asked in the interview if you spent the money on medical expenses. Answer "yes". Continue through the interview until you reach the page asking if you were covered by a high-deductible plan.  Here, indicate the period of time you were covered by an HSA.

I went through those steps and answered correctly as you shown, but it did not correct the issue.  What else could it be?

You must complete the HSA contribution section to establish your eligibility to have made the HSA contribution.
