The form for an HSA is the 8889. You should see form 8889-T in the list of forms, and 8889-S if you are married (even if your spouse does not have an HSA).
You make most of the form 8889 entries by going to the HSA interview. To do that, do a Search (upper right) for "hsa" (lower case, no double quotes) and jumping to the result.
Make sure you go through the entire HSA interview.
