Your HSA contribution that is done through payroll and reported in Box 12 of your W-2 does not appear anywhere on your Form 1040 because it is not included in your wages reported in box 1. In addition it is not included in your social security and Medicare wages. So, you don't get a deduction on your tax return because it has already been deducted from your taxable wages.
