Where on the tax form do you enter the actual fuel cost? There isn't a specific place for it.

I drove my motorhome to the Mayo clinic for treatment.  My fuel costs are much higher than the allotted mileage.  I will have a mixture of mileage and actual fuel cost.  There is a place to enter the mileage costs, but there is not a place on the Schedule A to enter just fuel costs.

When figuring your expenses for medical travel, you can either include your actual expenses or mileage, but not both for the same trip. 

You can include your actual travel expenses in the Other (bus, parking, taxi, etc.) field in the medical deductions section or go all the way to the end of the medical section where you can enter your own description and amount for the expense.  This page is titled Tell us about any other medical expenses.

Either way, all medical expenses are added together for one total.  The separate categories are there just to make it easier for you to enter everything without adding it yourself.
