Where do I find my HSA contributions on my W-2?

The HSA contribution is reported in Box 12 with code W on your W-2 form.

code w on form w-2 includes your contributions plus any made by the employer.  they all flow to form 8889 line 9. do not enter yours on line 2. this will double up your contributions.
