Where do I find my 1099-SA form?

If you took any distributions from your Health Savings Account (HSA) during the year, then your HSA account administratorwould issue the Form 1099-SA.  

If you did not take any money out of the account, then you wouldn’t be receiving a form for 2017.  If you have online access to your HSA account, you may be able to access the Form 1099-SA through that account.  

If you are not sure who the account administrator is, contact your employer or insurance provider.

If you want to change your address in TurboTax, please follow these steps:

1. Log into TurboTax and click on any topic to continue
2. Click on My Info from the menu on the left-hand side
3. Scroll down to Mailing Address and click on Edit
4. Enter your new address
